Missed a GLP-1 Dose? What the Labels Say

Missed a GLP-1 dose? See what the official FDA labels for Ozempic, Wegovy, Mounjaro, Zepbound, Trulicity and more say about missed doses and timing windows.

Life happens. A trip, a busy week or a pen left in the fridge at home can make you miss a GLP-1 dose. The good news is that every FDA-approved GLP-1 product has a written missed-dose rule in its official label, and those rules are clear once you know where to look.

Key takeaways

  • Each GLP-1 product has its own missed-dose rule. Do not borrow one product's rule for another.
  • Weekly injections use time windows such as 5 days (Ozempic), 4 days or 96 hours (Mounjaro, Zepbound) or "at least 3 days until the next dose" (Trulicity).
  • No label tells you to double up to make up for a missed dose.
  • After a longer gap, call your prescriber before restarting. Some labels describe restarting the dose schedule from a lower step.
  • Your own label and your prescriber's instructions come first. This article is a summary, not a substitute.

Why missed-dose rules differ

GLP-1 medications are different molecules with different half-lives and dosing schedules. Some are given once a week, some once a day, and some are tablets. Because of this, the FDA-approved prescribing information for each one has its own instructions for what to do after a missed dose.

The quotes below come from the official prescribing information and Medication Guides on DailyMed and FDA's label database, checked in October 2026. Labels get updated, so always check the Medication Guide that came with your pharmacy fill.

Weekly injections: what each label says

Ozempic (semaglutide injection)

The Ozempic injection label says: "If a dose is missed, administer OZEMPIC as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day." Patients then go back to their regular once-weekly schedule.

Wegovy (semaglutide injection)

The Wegovy label bases the decision on how soon your next dose is. If one dose is missed and the next scheduled dose is "more than 2 days away," take the missed dose as soon as possible. If it is "less than 2 days away," do not take the missed dose. The Medication Guide puts 2 days as 48 hours.

The label also covers longer gaps: "If 2 or more consecutive doses of WEGOVY injection are missed, resume dosing as scheduled or, if needed, reinitiate" and follow the dose-escalation schedule. That decision belongs to your prescriber.

Mounjaro and Zepbound (tirzepatide)

Both tirzepatide labels use the same window. The Mounjaro Medication Guide says: "If you miss a dose of MOUNJARO, take the missed dose as soon as possible within 4 days (96 hours) after the missed dose. If more than 4 days have passed, skip the missed dose and take your next dose on the regularly scheduled day."

The Zepbound Medication Guide uses the same 4-day (96-hour) rule and adds: "Do not take 2 doses of ZEPBOUND within 3 days (72 hours) of each other."

Trulicity (dulaglutide)

The Trulicity Medication Guide says to take the missed dose as soon as possible "if there are at least 3 days (72 hours) until your next scheduled dose. If there are less than 3 days remaining, skip the missed dose and take your next dose on the regularly scheduled day." It also says: "Do not take 2 doses of TRULICITY within 3 days of each other."

Quick comparison

Product Label's missed-dose rule (summary)
Ozempic injection Take within 5 days of the missed dose; after 5 days, skip it
Wegovy injection Take it if the next dose is more than 2 days (48 hours) away; skip if less
Mounjaro Take within 4 days (96 hours); after 4 days, skip it
Zepbound Take within 4 days (96 hours); after 4 days, skip it
Trulicity Take it if at least 3 days (72 hours) remain until the next dose; skip if less

Notice that two styles of rule exist. Some count days since the missed dose (Ozempic, Mounjaro, Zepbound). Others count days until the next dose (Wegovy, Trulicity). Reading your own label carefully avoids mixing them up.

Daily injections and tablets

Saxenda (liraglutide)

Saxenda is a once-daily injection. Its prescribing information says: "If a dose is missed, resume the once-daily regimen as prescribed with the next scheduled dose. Do not administer an extra dose or increase the dose to make up for the missed dose." It also says that if more than 3 days have passed since the last dose, Saxenda should be restarted at the label's starting dose and stepped up again, which your prescriber will guide.

Semaglutide tablets

The Medication Guide for Rybelsus and Ozempic tablets says: "If you miss a dose of RYBELSUS or OZEMPIC tablets, skip the missed dose and go back to your regular schedule." The Wegovy label says that if a dose of Wegovy tablets is missed, "skip the missed dose and take the next dose the following day."

What about compounded GLP-1s?

Compounded semaglutide or tirzepatide is not FDA-approved, so it has no FDA label with a missed-dose section. If you use a compounded product, ask the prescriber or pharmacy that supplied it for written missed-dose instructions. Do not assume a brand-name rule applies, especially if the concentration or schedule differs. Our guide on brand-name vs. compounded GLP-1s explains more.

After a longer break

Missing one dose is common. Missing several is different. GLP-1 doses are usually raised slowly to help the stomach adjust, which is why our titration guide matters here. Jumping back in at a higher dose after weeks off can bring back nausea and vomiting. Several labels describe restarting the escalation schedule after a longer gap. Call your prescriber before your next dose so they can tell you exactly how to restart.

Call sooner if you have severe stomach pain, ongoing vomiting, signs of dehydration or low blood sugar symptoms, whether or not you missed a dose.

Ways to miss fewer doses

  • Pick a fixed day and time. Tie it to something you already do each week.
  • Use reminders. A phone alarm or the PepSteady app can log each dose so you can see at a glance when your last one was.
  • Write down the date you missed. That makes the label's day-count easy to apply.
  • Plan for travel. Keep your pen in an insulated case and check the label's storage rules before you go.

Some labels let you change your weekly dosing day as long as doses stay far enough apart. For Mounjaro and Zepbound, that gap is "at least 3 days (72 hours)." Ask your pharmacist before changing your day.

The bottom line

Missing a GLP-1 dose is usually manageable, and the answer is printed in your product's Medication Guide. Find your product's rule, count the days carefully, never double up, and call your prescriber after a longer gap. When in doubt, your own label and your prescriber's instructions always come first.

Frequently asked questions

Is the missed-dose window the same for every GLP-1?

No. Each product has its own rule in its FDA label. For example, the Ozempic injection label uses a 5-day window, Mounjaro and Zepbound use 4 days (96 hours), and Trulicity and Wegovy injection base the decision on how many days remain until the next dose.

Should I take two doses close together to catch up?

No label tells you to double up. The Trulicity and Zepbound Medication Guides specifically say not to take 2 doses within 3 days (72 hours) of each other. If you are unsure, skip the guesswork and call your pharmacist or prescriber.

What if I missed several weeks of my GLP-1?

Do not restart on your own at your old dose. Some labels describe restarting with the dose-escalation schedule after longer gaps, and your prescriber needs to decide how you restart. Call them before your next dose.
